SOUL YOGA OHC WAIVER OF LIABILITY/INFORMED CONSENT: I have enrolled in a program of physical activity including but not limited to various yoga and meditation exercises offered by SOUL YOGA OHC. I am in good physical condition and do not suffer from any disability that would prevent my participation in this program. If I do have a physical condition, which limits my participation, I take full responsibility for modifying my activity and being aware of my own limitations. I have told my health professional about taking this program. In consideration of my participation with SOUL YOGA OHC’s exercise and health program, I, for myself and my family, release SOUL YOGA OHC (its employees,independent contractors, and owners), from any claims, demands, and causes of actions arising from my participation in the exercise program and I hereby release SOUL YOGA OHC from any liability now or in the future from any illness or injury, however caused, occurring during or after my participation in this exercise program.I agree that I am personally responsible for my safety and actions while using SOUL YOGA OHC. I agree to comply with all policies and rules, including but not limited to all SOUL YOGA OHC policies, guidelines, signage, and instructions. Because SOUL YOGA OHC is open for use by other individuals,I recognize that I am at a higher risk of contracting COVID-19. With full awareness of the risks involved, I, for myself and on behalf of my family, spouse, estate, heirs, executors, administrators,assigns, and personal representatives, hereby forever release, waive, discharge, and covenant not to sue SOUL YOGA OHC, its owners, agents, independent contractors, affiliates, employees, landlords, successors, and assigns (collectively the “Released Parties”) from any and all liability,claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, or injury, including death, that may be sustained by me related to COVID-19 whether caused by the negligence of the Released Parties, any third-party using SOUL YOGA OHC, or otherwise while participating in any activity while in, on, or around SOUL YOGA OHC and/or while using any SOUL YOGA OHC facilities, tools, equipment, or materials.
